Welcome to the website of Hoyt Integrative Health (“Hoyt Integrative Health,” “we,” “us,” or “our”). These Terms of Use (“Terms”) govern your access to and use of our website located at https://hoytintegrativehealth.com (the “Site”). By accessing or using the Site, you agree to be bound by these Terms. If you do not agree, please do not use the Site.
No Medical Advice
The content on this Site — including text, graphics, articles about conditions and services, and other materials — is provided for general informational and educational purposes only. It is not medical advice and is not a substitute for professional medical care, diagnosis, or treatment.
Reading this Site does not create a doctor-patient relationship. A doctor-patient relationship is established only through a formal consultation and agreement to provide care. Hoyt Integrative Health does not function as an urgent care or primary care provider.
Always seek the advice of a qualified healthcare provider with any questions you have regarding your health or a medical condition. Never disregard professional medical advice or delay seeking it because of something you read on this Site.
In a medical emergency, call 911 or go to the nearest emergency room immediately. Do not rely on this Site or on electronic communications with us for urgent or emergency medical needs.
No Guarantee of Outcomes
Functional and integrative medicine involves individualized, root-cause-focused care. Individual results vary from person to person, and nothing on this Site is a promise or guarantee of any particular health outcome. Patient testimonials and success stories reflect individual experiences and are not representative of, or a guarantee of, the results any other person may achieve.
Use of the Site
You agree to use the Site only for lawful purposes and in a manner that does not infringe the rights of, or restrict or inhibit the use of the Site by, any third party. You agree not to:
- Use the Site in any way that violates applicable law or regulation.
- Attempt to gain unauthorized access to any portion of the Site, its servers, or any connected systems.
- Introduce viruses, malware, or other harmful code.
- Use any automated means to access or scrape the Site in a manner that burdens our infrastructure, except as permitted by our published machine-readable files (such as robots.txt).
- Copy, reproduce, or redistribute Site content except as permitted below.
Intellectual Property
All content on the Site, including text, graphics, logos, images, and the compilation thereof, is the property of Hoyt Integrative Health or its licensors and is protected by United States and international intellectual property laws. The name “Hoyt Integrative Health,” the slogan “Real Health Restored,” and associated marks are the property of Hoyt Integrative Health. You may view and print content for your personal, non-commercial use only. Any other use requires our prior written permission.
Consultations, Scheduling, and Payment
Requests for consultations submitted through the Site are requests only and do not constitute a confirmed appointment until scheduled and confirmed by our office. The initial consultation is a discovery consultation to determine whether the practice’s functional medicine approach is appropriate for your needs; not all prospective patients will be accepted for ongoing care.
- Payment. Hoyt Integrative Health does not accept insurance. We accept cash, Care Credit, HSA, and FSA. Consultation fees are due at the time of service. Testing and program costs are discussed during the consultation, and you are not obligated to proceed with testing or programs after the consultation.
- Complimentary discovery consultations are limited to one per household. Minors must be accompanied by an adult.
- Cancellation. You must cancel or reschedule at least 48 business hours before your scheduled appointment to be eligible for a refund. Late cancellations may be subject to fees.
Telehealth
Telehealth services are provided using secure, HIPAA-compliant technology to patients located in jurisdictions where we are able to provide such services. By using our telehealth services, you acknowledge that telehealth has both benefits and limitations, and you consent to receiving care through this method. Telehealth informed consent is addressed separately as part of the care process.
Patient Portal and Third-Party Services
Certain features, such as appointment scheduling and the patient portal, are provided through third-party services that may be hosted on separate websites. Your use of those services is subject to the third-party provider’s own terms and privacy practices in addition to these Terms. We are not responsible for the availability, content, or practices of third-party services, though we maintain agreements with providers who handle protected health information as required by law.
Third-Party Links
The Site may contain links to third-party websites or resources. We provide these links for convenience only and do not endorse and are not responsible for the content, products, or services on third-party sites. Your use of third-party sites is at your own risk and subject to their terms.
Disclaimer of Warranties
The Site and its content are provided “as is” and “as available” without warranties of any kind, whether express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, secure, or free of viruses or other harmful components, or that the information on the Site is accurate, complete, or current.
Limitation of Liability
To the fullest extent permitted by law, Hoyt Integrative Health and its owners, providers, and staff will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of data, arising out of or related to your use of, or inability to use, the Site or its content. Nothing in these Terms limits liability that cannot be limited under applicable law, and this provision does not apply to the provision of medical care, which is governed separately.
Indemnification
You agree to indemnify and hold harmless Hoyt Integrative Health and its owners, providers, and staff from any claims, damages, losses, or expenses (including reasonable attorneys’ fees) arising out of your misuse of the Site or violation of these Terms.
Governing Law
These Terms are governed by the laws of the State of California, without regard to its conflict-of-laws principles. Any dispute arising out of or relating to these Terms or the Site will be subject to the exclusive jurisdiction of the state and federal courts located in California.
Changes to These Terms
We may update these Terms from time to time. When we do, we will revise the “Last updated” date above. Your continued use of the Site after changes are posted constitutes your acceptance of the updated Terms.

These Terms of Use are a general template and do not constitute legal advice. Hoyt Integrative Health should have this document reviewed by a qualified attorney before publishing, particularly the medical disclaimer, telehealth, limitation of liability, and governing-law provisions, and should ensure a separate telehealth informed-consent process and HIPAA Notice of Privacy Practices are in place.
